• Skull Wild: Substitutes for any other symbol except Scorpion, Spider, and Dead Patient. 
• Scorpion: Only found on reels 1, 3, and 5, these trigger the Autopsy Free Spins if you land 3 of them. Any Scorpion that lands in a Fire Frame in a bonus round awards an extra spin and makes that frame sticky.
• Spider: Lands on reels 2 and 4 and triggers the Mental Transform feature.

The base game is built on Fire Frames, which randomly split any symbol into two. But the real chaos starts when you land a spider symbol on reels 2 or 4. This triggers the Mental Transform feature, which turns that spider into a patient or an xMechanic.
